WHAT TO EXPECT ON MY FIRST VISIT AND KEYS TO A

GREAT EXPERIENCE AT TRI-STATE PHYSICAL THERAPY

All of our patients are very important to us, and we keep our patients’ best interests in mind with

every decision we make. In order to continue to provide the highest level of one-to-one care to

our patients, we had to make the difficult decision to only accept traditional Medicare and

TriCare or TriWest as of March 2, 202


One could get a referral from their healthcare provider, or may be seen directly if their insurance

does not require a referral (for example, traditional Medicare). Just give us a call or send a

message in the Contact section of our website (www.tsphysicaltherapy.net) to set up an

appointment. We are by appointment only. Please state your future availabilities and preferred

times of the day, and we will do our best to cater to your preferences. This will depend on our

existing patient volume and the for those who have special needs due to their medical condition


Once we get a referral and had preliminary verified your insurance, we call to verify more

information before setting up the appointment for the physical therapy evaluation. It is best to

always follow up with your referring provider to fax the order to (928) 248-0443, or for some

individuals who live close to our clinic, hand the referral over to us


Aside from general orthopedic and neuromuscular conditions, we specialize in chronic pain

conditions stemming from an old injury or experience in the past. You will be seen by a licensed

Board-Certified and Fellowship-trained Doctor of Physical Therapy with advanced training in

orthopedic manual physical therapy practice in a private room/space to discuss your medical

condition(s) and specific rehabilitation goals. We focus on pain science education, lifestyle

management/coaching, personalized exercise programming, and empowerment. We always

keep up to date with our approaches and offer what is evidence-based to all our patients. We

are usually the last stop when our patients have seen many practitioners in the past. If you

prefer to be in a physical therapy clinic with a lot of exercise equipment, constant social

interactions, and supervision, we are more than happy to refer you to another facility that

provides that

We offer the following advanced physical therapy services (when indicated):

  • spinal and joint mobilization/manipulation (not chiropractic)
  • myofascial decompression/cupping
  • dynamic integrative dry needling (not acupuncture)
  • microcurrent point stimulation (not your regular TENS unit)
  • transcutaneous vagal nerve stimulation (a non-invasive procedure which helps with chronic
  • pain, sleep, and stress


Good question. As of this time, most insurance carriers, including traditional or managed

Medicare, don’t cover dry needling (always check your PT benefits for the current insurance

provider). We offer this as an add-on cash service billed per visit when medically necessary and

safely indicated. How much? Not more than a usual copay would cost. Check in with us for the

As of March 2026, our clinic is at 1510 E Wagon Wheel Ln Ste 110, Fort Mohave, AZ

86426. It is inside the Tri-State Pain Institute at Suite 110, right next to Tri-State Primary

Care and Academic Urgent Care. The building is right next to Valley View Medical Center,

across E Wagon Wheel Ln, after turning from Highway 95. Head over to the front desk and

mention if you are a new patient or a returning patient, specifically for Physical Therapy. Be

prepared to present your Photo ID, insurance card(s), completed intake form (if completed at

home), and other important documents needed for the visit.


Yes. To prepare for your first visit, it is better to complete patient forms (print or electronic) ahead

of your appointment to save time (15 minutes earlier). Feel free to leave your best email address

in the Contact section of our website if you would like us to send your intake forms ahead of

time. Completing the form to the best of your knowledge helps us understand your condition

better


Bring your referral (highly recommended if available), insurance card(s), and photo ID to your

first appointment so our team can verify your benefits and coverage. If you have blood work

and/or imaging results available, and current medication list, it is recommended to bring them on

your first visit. You may also bring your assistive device or wheelchair (if you have one), bottled

water, and a face towel on your follow-up visits.


Your first visit will include an evaluation. This lets your provider assess your current health

situation, symptoms, and discuss your goals. This process can last up to an hour or so,

including paperwork, and might include treatment if necessary. Follow-up visits could take up to

an hour or less with a frequency of 1-3x/week, depending on many factors such as distance

from home, financial status, medical condition(s), transportation, etc. Depending on a specific

condition, we establish a plan of care averaging 2-3 months with the expectation that once could

sustain performing their exercise program at home and/or in a gym at the end.


You may bring one friend, family member, or caregiver with you to your evaluation. For future

visits, you’ll be seen alone to protect your privacy and the privacy of other patients. Minors

require a parent or a guardian for every visit


This situation happens every now and then. If your referring provider is not aware of your

scheduled surgical procedure that would greatly impact your participation in physical therapy, it

is good to inform them of this and discuss your potential options. If your physical therapy referral

is related to that surgical procedure (e.g., a prehab to strengthen muscles or joints before a

procedure), then one may proceed to see us for their evaluation

We don’t speak Spanish; however, a family member or friend may act as an interpreter only if

the patient requests it, understands that a free professional interpreter is available, and agrees

to use the personal interpreter. Let us know ahead of time if a professional medical interpreter

will be needed
